Criminal History Background Check Requirement

• CHBC needs to be current and submitted prior to Student Teaching. Send CHBC to Ms. Sherry Harvey, 13485 Cumberland Road, Fishers, IN 46038.
  1. Obtain fingerprint card and Form 8053 from the HSE Administrative Office.
  2. Take both of the above forms to your local police department and be fingerprinted.
  3. Mail fingerprint card, Form 8053, $10.00 certified check or money order (made out to The State of Indiana) and a self-addressed stamped envelope to the Indiana State Police Department.
     OR
     Go directly to the Indiana State Police Central Repository located at the Indiana Government Center North, 100 N. Senate Ave., Rm. N302, Indianapolis. They will need to see a picture I.D. (driver’s license) and will collect $10 cash, money order or cashier’s check. They will mail your background check to you within 24 hours.

Criminal History Background Check Requirement

• The CHBC Forms must be completed and filed 2 weeks prior to start of student teaching and current within 90 days.
• Non-Marion County Residents must complete the appropriate form for non-residents. Take it their local sheriff’s department and have it sent to IPS according to instructions on application. THIS MAY TAKE LONGER THAN 2 WEEKS SO PLEASE ALLOW SUFFICIENT TIME.

Criminal History Background Check Requirement

• Student teachers/field experience teachers must have two police checks before they will be allowed to work in the classroom.
• When student is accepted for student teaching, Perry mails a letter to student explaining police checks how and where to obtain them.
• These checks must be valid within the past year and submitted prior to commencing student teaching.
  1. Indiana State Police Check-- limited criminal history check. Indiana State Police Central Repository located at the Indiana Government Center North, 100 N. Senate Ave., Rm. N302, Indianapolis
  2. Indianapolis City Police Check—includes fingerprinting. Indianapolis City County Bldg.
